Referrals

Care Assurance is able to accept both planned and emergency referrals for adults with learning disability and/or mental health issues, and young people leaving care.

Based on the agreed outcomes and following our robust assessment process, we will always aim to ensure that the chosen accommodation and agreed support and care plan meets their initial needs and continues to be reviewed and adjusted so that our services continue to meet our clients changing needs.
